As a student of Mater Dei Catholic School, your child’s image may be used in a photo or video posted on: • School website and/or communications • Archdiocesan or Office of Catholic Education website and/or communications • Parish website and/or communication • Local news media concerning events related to the school and its programs When photos are used in almost all cases last names are not included. First Names will be included with photos rarely and only in cases where there is special recognition such as with academic or sport awards/recognition. Your child’s safety and security are our number one priority. All websites and tools have been and will continue to be thoroughly examined by parish and school administrators. A separate form MUST be submitted for each child in a family if you are opting out as a family. ALL starred fields MUST be completed in order to submit your form.
